I may be wrong, but in parsing, what I take from this post-game presser account from Ira Winderman is that there is a reciprocal lack of belief between players and coach. Players have had issues with Spoelstra this season. His two brain cramps against Detroit, Bam yelled at him, Hi-Ho was puzzled and miffed at going from starter to DNPCD; Spo trusts Kel'el to start--but not finish games. There seems to me lack of buy-in by players and a lack of faith by the coach in some players.

That's it. Similar to what happened, inexplicably, to Manchester City this season. Failure happens of course, but unless you're mentally strong enough to maintain belief, it "permeates" everything.
